Laurel Hill Center is seeking a full time Peer Support Specialist to join our ACT Junction City team.

The Peer Support Specialist is a member of the Assertive Community Treatment team, providing individual skills training, case management, and other supports in the office, home or community to participants to live in the community, work, establish positive supports, and meet other recovery goals identified by the individual.  LHC mission and values stipulate that all services provided are based on participant strengths and choices and follow evidenced based practices of psychosocial rehabilitation.

Essential Functions:

 

  • Assist participants in Assertive Community Treatment develop skills, build confidence and competence in using natural support networks and accessing community resources required for independent living such as grocery shopping or bill paying
  • Assist participants in attending appointments
  • Explore and support participants in community activities to reduce social isolation
  • Support participant in building relationships with others
  • Use individual and/or group mentoring and modeling to assist participants in improving their quality of life
  • Help participants develop use of recovery strategies to manage symptoms, plan for daily wellness activities, and develop crisis management plans
  • Clearly and consistently communicate with program staff about the needs, experience, and observations of program participants and ways to improve the program to support participant success
  • Participate in agency and staff team meetings
  • Complete necessary documentation using a computer in a timely and accurate manner
  • Other duties as assigned by Team Leader or Supported Housing

  • Each member of the ACT team is required to work one evening or weekend shift per week and will be part of an on call rotation for the crisis line in case an ACT participant calls.

 

Minimum Requirements

  • Peer Support Specialist certification, including completion of state approved 40 hour PSS training course and Traditional Health Worker registry number, or
  • Qualified applicants who meet requirements for QMHA credential will also be considered and must be willing to get their Traditional Health Worker registry number within six months of the start of employment.
  • Must have current and valid Oregon driver's license
  • Experience and willingness to identify oneself as a person with experience as a consumer of mental health services, particularly individuals whose lived experiences commensurate with the participants of LHC services (individuals who have been diagnosed with a severe mental illness and whose symptoms significantly impacted their ability to live, work or learn for over 12 months).
  • Documentation of successful completion of a background and fingerprint check as specified by Oregon law as well as a urinalysis drug screen
  • Ability to use a computer, cell phone and e-mail for documentation and communication
  • Must be able to manage the physical demands that include getting in and out of vehicles, climbing stairs, and lifting/carrying 25 pounds or under
